Business Associate Agreement (BAA)

This Business Associate Agreement (“BAA”) is entered into between ClearPath Payer, Inc. (“Business Associate”) and the customer entity (“Covered Entity”).
1. Permitted Uses and Disclosures
Business Associate shall not use or disclose Protected Health Information (PHI) or provider Personally Identifiable Information (PII) other than as permitted or required by this Agreement or as Required by Law.
2. Safeguards and Incident Reporting
Business Associate implements administrative, physical, and technical safeguards that reasonably protect the confidentiality, integrity, and availability of electronic PHI in accordance with 45 C.F.R. § 164.308.
